Acknowledgment of Responsibility and Risk
I, the parent or legal guardian, understand that participation in the literacy classes hosted by Dr. Deanna Learning for Littles involves activities that may carry inherent risks of minor injuries such as bumps, bruises, or falls, which are common during toddler play and interaction.
I acknowledge and agree to the following:
Parental Supervision is Mandatory: I understand and agree that I must remain present and within close proximity to my child at all times during the event. I am solely responsible for supervising, monitoring, and ensuring the safety and behavior of my child while participating in any activities organized by Dr. Deanna Learning for Littles.
Assumption of Risk: I voluntarily assume all risks associated with participation in the group activities, including but not limited to risks of physical injury, emotional distress, or accidents that may occur during participation.
Waiver of Liability: I hereby release, waive, and discharge Dr. Deanna Learning for Littles, its owners, employees, volunteers, and affiliates from any and all liability, claims, demands, causes of action, or losses for bodily injury, property damage, or other harm that may occur to me, my child, or any other accompanying persons arising out of or related to participation in the group.
No Childcare Provided: I understand that Dr. Deanna Learning for Littles does not provide childcare services. This is not a drop-off program. I remain fully responsible for my child’s safety, behavior, and well-being at all times.
