Acknowledgment of Responsibility and Risk
I, the attendee, understand that participation in the literacy classes hosted by Dr. Deanna Learning for Littles involves activities that may carry inherent risks of minor injuries such as cuts while making materials. The participant is responsible for the careful and correct use of materials.
I acknowledge and agree to the following:
Acknowledgement of Photo and Video-taped Events: This event may at times be video-taped or photographed for the intent of parent-education and to promote the activities of Dr. Deanna Learning for Littles, including trainings and social media usage. I understand that my participation in this session is an agreement to this. No names, identifying factors, or private information is ever disclosed.
Mom event (the kids stay home this time): I understand and agree that this event is for grown-ups and the sole purpose of it is for parent- education and fellowship for moms
Assumption of Risk: I voluntarily assume all risks associated with participation in the group activities, including but not limited to risks of physical injury, emotional distress, or accidents that may occur during participation.
Waiver of Liability: I hereby release, waive, and discharge Dr. Deanna Learning for Littles, its owners, employees, volunteers, and affiliates from any and all liability, claims, demands, causes of action, or losses for bodily injury, property damage, or other harm that may occur to me, my child, or any other accompanying persons arising out of or related to participation in the group.
No Childcare Provided: I understand that Dr. Deanna Learning for Littles does not provide childcare services.
